This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

The Heart of America Health Plan (HAHP) values you as a member. That is why we would like to take this opportunity to describe our privacy policies and what we do to protect your personal health information.

You have provided us with most of the information we use for determining the acceptance of your application during our underwriting process. If more information is needed, we may collect non-public personal health information about you for determining acceptance and/or servicing your policy with the Heart of America Health Plan.

We do not disclose any non-public information about our customers or former customers to anyone, except as permitted by state and federal law.

We restrict access to information about our members to those employees who need to know such information to provide you with benefits or service your policy. We comply with the state and federal regulations that guard this non-public information about you.

You have the right to look at medical records held at Heart of America Health Plan, and to get a copy of your records. You also have the right to ask us to make additions or corrections to your medical records.
