Rugby Service Area
Any subscriber and eligible dependents, who agree to receive primary
care through their primary care physician and reside within the
service area, are eligible for membership. The communities within the

Dependent Eligibility
Eligible dependents include a subscriber’s spouse and each of the
subscriber’s and/or spouse’s unmarried dependent children who reside
with the subscriber until the end of the month of which the dependent
turns 22. Full-time students who reside with the subscriber, except
while attending school, are covered until the end of the month of
which the dependent turns 26.

Facts About Heart of America Health Plan
This plan is a comprehensive medical plan called a Health Maintenance
Organization. It has features that you must understand before deciding
whether it is the right choice for you and your family. Unlike
traditional insurance, which pays claims after care is rendered, HAHP
arranges in advance with doctors and hospitals to give care to members
and pays them directly for their services.
Primary
Care is available only from physicians of the Johnson Clinic, P.C.
in the Rugby Service Area and the Trinity Clinics in the Minot
Service Area (called Plan Physicians) except during a medical
emergency.
Because HAHP provides care and pays the cost, it seeks efficient and
effective delivery of health services. By controlling unnecessary or
inappropriate care, it can afford to offer a more comprehensive range
of benefits than many indemnity insurance plans.
In addition to providing comprehensive health services and benefits
for accidents, illness and injury, HAHP places a great emphasis on
preventive benefits such as office visits, physicals, immunizations
and well-baby care. Members are encouraged to get medical attention at
the first sign of illness.
Heart of America Health Plan (HAHP) is a Health Maintenance
Organization with a strong interest and commitment to member
participation. HAHP is staffed with people who listen to your medical
needs.
As a member you also have a say in the administration of Heart of
America. The HAHP Board of Directors, who set overall policies for the
plan, are elected to serve rotating terms. The Board is made up of
eleven consumers and providers of health care from throughout the
market area.
When you want to use the Plan’s preventive services or need medical
care, simply call your clinic office and make an appointment. As a
member of HAHP, you agree to receive your primary care from one of the
Plan Physicians. Your Plan Physician will provide for, arrange and
coordinate your health care needs, including, when necessary,
referrals to specialists to meet your individual needs.
In addition to these services, HAHP provides a complete range of
hospital and clinic services to cover the costs of illness and
hospitalization.
